Counter-Notices

If you believe that your material was removed in error, you may submit a counter-notice. This counter-notice should also be sent to dmca@formstemplates.net and must include:

  • A description of the material that was removed and the location from which it was removed.

  • A statement asserting that you have a good faith belief that the material was removed due to a mistake or misidentification.

  • A statement, under penalty of perjury, that the information in your counter-notice is accurate.

  • Your electronic or physical signature.
